A service for children who have completed an OT assessment showing weakness in multiple areas of function but not a significant developmental delay or impairment yet continue to struggle functionally at home, school or in the community. We believe children shouldn’t need a medical diagnosis in order to access supportive services and that supporting those who support children is one way of making life easier for children and families.
Service includes direct time with parent(s), teachers or others important to the life of the child, in order to provide clinical brainstorming and strategies that may support improved levels of function based on the results of the assessment for that child. Up to the equivalent of 3 or 6 hours of support will be provided over the duration of the package.
Service also includes collaboration with other disciplines as needed to support carryover and integration of OT strategies into the child’s other treatment plans. This may include direct meetings (IEPs, or care coordination), phone calls or email correspondence. Up to the equivalent of an hour of service will be provided intermittently as needed throughout the duration of service. Time may be used to attend a meeting as long as it is local and as mutually agreed upon by parents and therapist.
